Go Green! - E-Statement Enrollment Form

Greetings from the Finance Team! In a stewardship effort to reduce our printing and paper consumption, we are launching an e-statement campaign in which Friendship givers may elect to receive their contribution statements electronically via email. If you would like to enroll yourself and/or your family in this ‘green’ statement option, please complete the enrollment form below.

If you have already requested that we send you electronic statements, please enroll as well so that we can include you formally in this e-statement campaign. All online submissions will be forwarded to the Finance Team.

This partnership between stewardship and technology creates a one-click scenario in which you can update your family’s contact information, login and view your giving history, enroll in our ‘green’ statement option, receive important text messages from your church and more using our newest web feature, Shelby Web View launching in June 2010!
